How should I prepare for my first appointment?
Prior to your first appointment please fill out all your paperwork and submit online. Forms will be emailed to you once you schedule your first visit.
Please email a copy of your insurance card (front and back) and doctor’s referral or prescription from your referring provider if you have one.
Please bring or email any medical records (chart notes, imaging or post operative protocols) that you think would be helpful for us to know.
Wear comfortable, loose fitting clothing. If we are working on hips or knees please plan to bring or wear shorts. If we are working on neck and shoulders please plan on wearing a tank top or a top that will allow us to easily see your neck and shoulders.
If you are coming for a running or ice skating assessment please bring running shoes or ice skates.
How long will my appointment last?
Most appointments will be between 45-60 minutes.
Do you accept insurance? Medicare?
Yes, we are in-network with most major insurance plans.
We are out of network with United HealthCare and Kaiser (except PPO plan).
We do not accept Medicare and our unable to provide out-of-network services to Medicare holders. Please contact us to discuss options if you are covered by Medicare.
Once you schedule an appointment and provide your insurance information we will verify your insurance benefits and provide you with this information after your initial visit. It is always a good idea to call your insurance company and verify your benefits yourself before hand. Simply call the number on the back of your insurance card or search for in-network providers on their website.
What is your cancellation policy?
We require 24hrs notice when canceling or rescheduling an appointment.
If cancel less than 24hrs or no showing an appointment could result in a $50 fee which will be due prior to scheduling another appointment.
Cancellations can be done via email, phone, text or online scheduling portal.
